Job Description

Post-Discharge Clinic (PDC) Responsibilities

  • Part-time position
    • Monday, Wednesday, Friday 9a-5p
  • Conduct follow-up visits (in-clinic, home, or via telehealth)
  • Perform physical exams
  • Review medications and new health conditions
  • Collaborate with physicians and other healthcare team members to ensure a smooth transition to the next phase of care
  • FNP, ACNP, AGACNP, and PA accepted

Facility Information

Baptist Health-Fort Smith is the oldest hospital in Arkansas, having originally been built in 1887. Since then, the Fort Smith hospital has expanded at its current location on Towson Avenue. Baptist Health-Fort Smith, which serves residents of the River Valley, joined Baptist Health in November 2018 and has 492 licensed beds. The nationally recognized, faith-based hospital is just south of downtown Fort Smith. Baptist Health-Fort Smith's Emergency Department is a Level III trauma center, which means it has demonstrated the ability to provide advanced trauma life support (ATLS) prior to the transfer of patients to a higher-level trauma center. Comprehensive Services include: Cardiology, Cancer Care, Diabetes Care, Diagnostic Imaging, Digestive Health, Emergency Services, Fitness Center, Heart Care, Geriatric Services, Kidney Care, Laboratory Services, Neurology, Neurosurgery, Orthopedic Care, Pediatric Care, Pulmonary Medicine, Rehabilitation Services, Sleep Medicine, Stroke Care, Wound Care, Urogynecology, Urology, Women's Health, and Labor & Delivery.
